Description

This dataset comprises the case note audit data submitted by hospitals that participated in second round of the National Audit of Dementia 2012-13. The audit was open to all general hospitals in England and Wales providing acute services on more than one ward and admitting adults over the age of 65. The second round of audit examined the quality of care received by people with dementia admitted to general hospitals between September 2011 and February 2012. Case notes were audited against a checklist of standards that relate to the patient’s admission, assessment, care planning and delivery, and discharge. All acute Trusts/Health Boards in England and Wales registered one or more sites to participate, and 98% of eligible hospitals participated in the audit. Only English data are presented in this dataset, which includes data from 190 hospitals that submitted a total of 7380 case notes. Sample size: Each participating hospital was requested to submit a sample of a minimum of 40 case notes. Please note that ‘not applicable’ responses were excluded from the analysis of questions, this may affect the number of case notes included in the analysis of individual questions. Questions presented in the ‘discharge from hospital’ section were not applicable to all patients in the sample (e.g. patients who died or were on end of life pathway) – a separate column presenting the sample size for each hospital in this section has been provided.

The dataset comprises the data submitted for the second round of the National Audit of Psychological Therapies for Anxiety and Depression (NAPT) during the period July 2012 – January 2013. This includes a retrospective case record audit, a therapist questionnaire and a service user questionnaire. The audit was open to all NHS-funded psychological therapy services in England and Wales providing therapy in the community for adults with anxiety and depression. The 220 services that participated constituted a wide range of psychological therapy services, including primary and secondary care, IAPT and non-IAPT, NHS managed, private or voluntary sector managed, and of varied size. (IAPT services are those that have been set up under the Improving Access to Psychological Therapies programme). The data show performance against the ten audit standards, which map onto four areas of quality: access, appropriateness, acceptability and outcomes. Only English data are presented in this dataset, which includes data from the 220 services which submitted a total of 122,812 case records, 4,661 therapist questionnaires, and 14,587 service user questionnaires. For each audit standard, where services had fewer than 30 eligible cases, it has been indicated that the number of cases is so small that the data are not presented for reasons of reliability.

This dataset comprises a set of analyses concerning the treatment of patients with heart failure by NHS hospitals in England in 2011/12. It has been reproduced from the National Heart Failure Audit 2011/12 Annual Report. The National Heart Failure Audit monitors the treatment of heart failure patients in England and Wales. It collects data on patients discharged from hospital with a primary diagnosis of heart failure. The audit covers all hospitals in England and Wales which admit acute patients with heart failure – the vast majority of hospitals currently submit data. Only English data are included in this dataset. Analyses are included for every eligible English hospital which submitted at least 100 patients, or 70% of all of their heart failure patients, to the audit. The analyses included: Number of records submitted; % of patients receiving an echocardiogram; % treated on a cardiology ward; % prescribed ACE inhibitor/ARB on discharge; % prescribed beta blocker on discharge; % referred for heart failure nurse led follow-up; % referred for cardiology follow-up.
